A parent or legal guardian must complete and authorise this application.

Queen Mode releases a Queen only to a Parent or Guardian or person recorded as authorised, unless an alternative arrangement has been expressly confirmed. Families must promptly update collection, custody, access and emergency information.

Previous experience is not required and does not determine age group or squad placement by itself.

This information is collected to help Queen Mode provide a safe and appropriate training environment.

Queen and contact details from earlier sections are summarised here without duplicate entry. Check or correct them in 1. Queen details and 2. Parent / Legal Guardian.

Parents and Guardians are responsible for providing Queen Mode with accurate and current information relevant to the Queen’s ability to safely participate. Queen Mode should be notified promptly of any significant changes to the Queen’s health, circumstances or ability to participate.

Sensitive information is collected only to support safe participation, emergency response and enrolment administration. See the Privacy Policy.

Medical clearance

Queen Mode is not a medical provider and cannot determine whether a Queen is medically fit to participate. Where a relevant condition, injury, restriction, recent illness or other concern may affect safe participation, medical clearance may be required before participation.

I confirm that, based on the information available to me, the participant is medically cleared to participate subject to any conditions listed below.

Each acknowledgement is recorded separately. Policy versions and effective dates are TBC until each document is issued; the printed copy carries the versions current at signing.

Queen Mode may communicate with Parents and Guardians regarding program information, training schedules, changes to sessions, attendance, administrative matters, program updates, important safety information and Queen progress where appropriate.

Queen Mode may capture photography, video or other media for legitimate program, promotional, educational or record-keeping purposes. Program participation does not automatically mean consent to public use of an identifiable image.

Content will be age-appropriate, respectful and consistent with safeguarding standards. A Queen must not be pressured to participate. This consent does not permit Queen Mode to disclose unnecessary personal information about the Queen.

The consent record uses the Queen’s full legal name from 1. Queen details and the Parent or Guardian full name from 2. Parent / Legal Guardian; they are not asked for again.

You may contact Queen Mode to change consent for future use. The final policy will explain practical limits for material already published.

In an emergency, Queen Mode will take reasonable steps to contact the Parent or Guardian or nominated emergency contact. Where immediate medical attention is reasonably considered necessary, I authorise Queen Mode staff to seek appropriate emergency medical assistance for the Queen.

I understand that Queen Mode operates on a paid registration and training basis.

I acknowledge that:

• Program fees are payable in accordance with the applicable Queen Mode pricing and payment terms.
• Places may be limited.
• Registration does not necessarily guarantee placement in a particular squad or session until confirmed by Queen Mode.
• I am responsible for ensuring fees are paid by the applicable due date.
• Queen Mode’s cancellation, withdrawal, refund and payment policies apply to my registration.

No payment is collected by this form. Rising is $50 per session, Development $60 per session and Performance $70 per session. Weekly and 10-week Term commitments depend on frequency. Places and schedules are not confirmed until Queen Mode accepts an application and confirms enrolment.

Parent or Guardian declaration

I declare that the information provided is accurate and complete to the best of my knowledge. I confirm that I am the Queen’s parent or legal guardian and have authority to provide consent for her to participate in Queen Mode. I understand that physical training involves inherent risks and that the Queen must follow reasonable safety instructions. I will notify Queen Mode if relevant information changes. I have read and agree to the applicable Queen Mode terms, policies and Codes of Conduct.

Queen declaration

I understand that Queen Mode is a place to learn, train, develop and grow. I agree to give my best effort, respect myself and others, listen to my coaches, take responsibility for my behaviour, support other Queens, respect the Queen Mode environment, follow the Queen Code of Conduct and keep my phone or device away during sessions unless authorised.
