These Terms & Conditions establish the framework for participation in Queen Mode Academy.

Fees and program structure

Rising is $50 per session, Development $60 per session and Performance $70 per session. Weekly and 10-week Term commitments depend on frequency. Places and schedules are not confirmed until Queen Mode accepts an application and confirms enrolment.

Media consent is separate; enrolment does not imply consent to public use of identifiable imagery.

Consumer rights

Nothing in Queen Mode’s Terms & Conditions is intended to remove rights that cannot legally be excluded under Australian law.

This agreement is intended to operate in Western Australia and must be applied consistently with applicable Western Australian and Commonwealth laws, regulations and safeguarding obligations.

Parent or Guardian Responsibilities

  • Provide accurate Queen (participant), emergency, medical and authorised-collection information.
  • Ensure suitable clothing, footwear, hydration and required equipment.
  • Notify Queen Mode of relevant health, injury or circumstance changes.
  • Ensure the Queen follows applicable policies and venue rules.
  • Respect Coaches, staff and other families.
  • Use the complaints process rather than disrupting sessions.

Queen Responsibilities

  • Attend prepared and on time.
  • Follow safety instructions and participate within individual capacity.
  • Treat others respectfully.
  • Report injury, illness, unsafe behaviour or concerns.
  • Follow device, media, attendance and conduct requirements.

Program Responsibilities

Queen Mode will seek to provide professional, age-appropriate training and a safe operating environment within reasonable limits. No physical activity program can eliminate every risk.

Risk and Acknowledgement

Participation carries inherent physical-activity risks, including injury, illness, falls, collisions and other events, and the Parent or Guardian acknowledges these risks on behalf of the Queen. The risk, insurance, cancellation, privacy and program terms set out in this agreement and any completed enrolment form form part of the Queen’s enrolment and participation conditions.

Insurance, Cancellation and Withdrawal

  • Queen Mode may maintain insurance relevant to its operations; however, the Parent or Guardian is responsible for considering any personal accident, ambulance, medical or private health cover required for the Queen.
  • Refunds will not be provided without reasonable cause; however, where a Queen misses a session due to illness or injury, Queen Mode may, where practical, offer a make-up session or apply a session credit in accordance with program availability and enrolment terms.
  • Participation is encouraged whenever the Queen is able to attend, and Queen Mode will seek to offer reasonable accommodations, modified activities or suitable alternatives to support continuity, engagement, community connection and team spirit.
  • The Parent or Guardian consents to Queen Mode using Queen, emergency, medical and authorised-collection information for enrolment, program delivery, safety management and emergency response where reasonably required.
  • Queen Mode may record, use and disclose relevant information where reasonably necessary to manage safety, respond to injury or illness, comply with law, or escalate safeguarding concerns to appropriate authorities or services.

Acknowledgement

By signing this agreement, the undersigned confirms they have read, understood and accepted the responsibilities, risk acknowledgement, insurance and cancellation terms, consent, privacy and safeguarding provisions that apply to participation.

The Queen Mode application records acceptance of this agreement as the Terms & Conditions acknowledgement, together with the Parent or Guardian and Queen declarations.
